It is impossible to say whether it is causation or socioeconomic or both. I do know there is definitely a relationship by observation over the years.

Before I looked at your subtext, my immediate thought was that I wouldn't waste too much time looking at causality on a general basis, though there are those who seem enamored of the idea of inflammatory markers in CHD. On the other hand, there certainly must be some causality in certain subgroups, and it doesn't take a lot of research to confirm that. Just look at the teeth of a few meth-heads! (But that isn't the causality you had in mind, is it?
