Diabetes is a blood vessel disease essentially. It affects all the blood vessels in the body. High blood sugar level is related to increase cholesterol and fat in the blood vessels, and these cholesterol and fat will stuck on the blood vessel wall, including the coronary arteries. Diabetes also causes calcium deposition on the blood vessel walls, which render the blood vessels wall to be thickened and inelastic and brittle. In the mean time, these blood vessels would become smaller in size in general. Gradually, the small, thick-walled, brittle, and full of cholesterol plague blood vessels develops. We called it coronary artery disease, or coronary heart disease, etc.. Cigarette smoke has many chemicals in it, in which tar can deposit onto blood vessel wall and cause similar changes in diabetes.

Smoking is a major risk factor for the development of heart disease. Cigarette contains nicotine. Nicotine is a potent vasoconstrictor, meaning that it narrows the passageway of your blood to be transported all over the body. In connection to diabetes, diabetic people have viscous (thick blood) due to high blood sugar levels. Thick blood can cause thrombus or what we call a clot. With continued use of cigarettes, (as blood vessels constrict, pressure increases) the clot can be dislodged and travel through the blood stream and can block the passageway of smaller arteries e.g. coronary arteries. The traveling clot which is now called an embolus impedes the blood flow to the coronary arteries since it blocks the passageway. Coronary arteries are the only arteries that supply oxygenated blood to the heart per se.
