Yes, you can have heart disease without hypertension. You could have a fatty heart, or a heart where the tissues are inflamed or dying making it difficult for the heart to pump blood. Smoking causes many of those conditions.

Seems absolutely plausible to me. But why would a 21 year old want to die? That's the most important question. Please contact a doctor immediately.
3 :
Yes. But smoking is your worse problem.

My Blood Pressure has never been any more than normal, and I have been suffering Heart disease for a minimum of 10 years now. Part of My treatment program involved actually lowering My blood pressure and slowing the heart down just a bit..

Three packs per day will kill you just like you are feeling. You are poisoning your heart and expecting it to work efficiently. Yes there are many heart diseases without a high blood pressure.

Stop smoking. If you can't, consult a physician (pulmonary medicine). Please also conduct your cholesterol profile test. There are several heart patients without hypertension. Such heart diseases are as follows:- HEART ATTACK HEART FAILURE ANGINA HARDENED ARTERIES DISTURBED HEART RHYTHM HEART MUSCLE DISEASE HEART VALVE DISEASE CONGENITAL HEART DEFECTS Adopt DASH diet. Eat a low-cholesterol, low-fat diet, which includes cottage cheese, fat-free milk, fish, vegetables, poultry, and egg whites. Use monounsaturated oils such as olive, peanut, and canola oils or polyunsaturated oils such as corn, safflower, soy, sunflower, cottonseed, and soybean oils. Avoid foods with excess fat in them such as meat (especially liver and fatty meat), egg yolks, whole milk, cream, butter, shortening, pastries, cakes, cookies, gravy, peanut butter, chocolate, olives, potato chips, coconut, cheese (other than cottage cheese), coconut oil, palm oil, and fried foods.

Yes, there is such a thing. If you must worry, worry about your tobacco habit. You may not 'go like that', but lung cancer is no picnic either. Lasts a bit longer than a heart attack. Your fatigue sounds like the result of smoking, depression, anxiety. Talk to your doctor, the only one who can see you, ask questions and check your BP accurately.

Plsssss...you don't wanna die, really? You're right that you should be worried about it because it's about your health and your life. Remember that your soul is precious to live another day. First of all, you must take care yourself well if you still love your life. Nobody wanna die at young age. I think you should check your blood pressure often because it can tell you how your heartbeat is, safe or dangerous condition. The easiest way to check it yourself (if you are lazy to see your doc or have no time) is find a blood pressure monitor to have it measured at home. It's safe and easy to do it daily. It's very useful to check if it is at normal level or not. Try it and you will feel better! Hope this might help you
